I've set up a fresh install of 1.5.0RC2 on Jooma 1.5.7.

Everything seems to be working *except* that I can't figure out how to add moderators to a forum. At the bottom of the page it says

Please do note: After setting Moderation to "Yes" you must save the forum configuration first before you will be able to add Moderators using the new button.

Where can I find this "new" button?

FWIW, I am working as a super administrator, and the Admin Access Level is set to "Public Back End". I have tried changing to match super admin, but that didn't change anything.

I have the same problem in 1.0.4 using Joomla 1.0.15.

Nope - doesn't work! It appears you can't administer this from here. No button appears or anything else after following the instruction to Save the configuration.

What I did to fix this (or at least find an alternative place to select or deselect moderators) is in User Administration. You'll find this at the FB Admin backend in the list of menu options to the left.

Find the user you want to select (or deselect) tick the box and click Edit. Here you'll now see the options relating to that user, and the relationships with any forums. Make your choices here. After Save, you should see that change reflected in the forum.

Works for me!
